2. From the list of displayed form types, select a form type depending on the type of desired output (PDF or questionnaire).

3. Select a component for which the survey is created.

4. Tap Apply.

• The survey opens as a PDF or a questionnaire depending on the form type.

5. Perform the required edits and save.
